Congratulations on applying for the INCOSE Equivalency from the U.  This form should be completed in the semester that you are finishing the requirements to be on the U’s INCOSE Equivalency List.  This does not earn you the ASEP or CSEP, but merely completes the INCOSE Knowledge Exam requirement.  You still need to apply for the appropriate certification with INCOSE (www.incose.org).
